Hello! I came here hoping to have found an answer to this. But! Now, I'm back, as I have now found a way!
I have all the FG annotations in one layer, but from multiple import files. The CAD tech or whoever created the file used the same names for layers regardless of phase 1 or phase 3, which caused the same problem for me.
Here's what I did: I selected everything on that layer, all the CAD text and arrows, and boxes. Under the macros tab, in the CAd section is a command called "convert to linestring" After that I re-ran project cleanup and removed duplicate items. BAM!!!! 697 duplicate items removed.
Not sure if you ever found an alternative way or not, but here is an option. especially for those who come across your post.

I think that the project cleanup tool is brilliant and use it a lot, but one of the things I wish it would do is remove duplicate CAD Text and block definitions.
The problem i have is that a lot of the CAD imports I do, especially for updates to engineering layouts, import into the same layers as the previous iteration. The duplicate lines get removed using the project cleanup but the duplicate CAD text and blocks stay.
I don't like deleting the previously imported version of the CAD file because quite often I have done hours or even days worth of work using the information.
